What pillow height is best for neck support?

Short answer. There is no universal height. Side sleepers generally need to fill more space than back sleepers, while shoulder width, mattress sink and pillow compression change the height that is actually experienced.

Useful height is the height under load

A pillow measured on a table does not tell you exactly how high it will support your head. Soft memory foam, springier latex and loose fibre can share the same stated thickness yet compress differently once you lie down.

The mattress also matters. If the shoulder sinks deeply when side sleeping, the remaining gap to the head becomes smaller. A firmer mattress can leave a larger gap.

Side sleeping: shoulder dimensions matter

A 2025 study tested nine healthy adults on foam-latex pillows measuring 8, 10, 12 and 14 cm. An index incorporating shoulder width strongly predicted cervical slope, and individualized medium height with neck support produced a cervical curve closer to standing and lower simulated musculoskeletal forces. The useful lesson is not that everyone should buy one particular centimetre value. It is that the person's body dimensions, mattress and material compression should be considered together.

Back sleeping: avoid both excessive flexion and inadequate support

When lying on the back, a very thick pillow can push the chin toward the chest. An extremely thin pillow may allow the head to fall back or feel unsupported. Comfort and head position need to be assessed together.

How to test height without chasing a magic number

  1. Lie in your usual sleep position.
  2. If possible, have someone look at the head-neck-torso line or use a side photo as a rough reference.
  3. For side sleeping, if the head clearly drops toward the mattress, test slightly more support.
  4. If it is pushed toward the ceiling, test slightly less.
  5. For back sleeping, watch for the chin being strongly pushed toward the chest.
  6. Give each small adjustment several nights while changing only one variable.

Why adjustable pillows are useful for testing

Removable inserts or fill make it easier to change height without immediately buying another pillow. That does not guarantee better outcomes, but it makes experimentation easier.
